Referral information – All that is needed for a referral is the patient demographic information and diagnosis, contact information of their current physician, and the best way to contact the patient or caregiver. At Agape, we are happy to assist with securing physician orders as well.
We respect your time and we are committed to streamline the referral process as much as possible. A simple call to 404.763.1456 or fax to 866.682.6164 to the Agape Hospice Care Intake Department, staffed by RNs, is all it takes.
Response time – At Agape, we respond to referrals within 12 hours and usually see the patient within 2-3 hours at the doctors office, hospital, assisted living facility, long term care setting, 55+ or home, from the time that you call, email, or fax in your referral.
We are able to evaluate the patients in the following locations: doctors office, hospital, or patients residence – whether that is a private home, assisted living facility, extended care unit, rehab department, 55+ senior care, or long-term care facility.
The earlier we are able to assess the patient to see if our program will best serve them, the quicker we can begin providing care, 24 on-call support, and provide supplies, medications, as well as set-up equipment if needed. This can be a cost-savings to the family.
Interdisciplinary Team – The Agape Hospice Care Team is made up of RNs, Certified Nursing Assistance, 24 – Hour On-Call Staff, Hospice Physician, Social Workers, Chaplains, Volunteers, as well as regionally based case managers who oversee the team, patient, and family who they are serving.
24 Hour Continuous Care We provide home care, in the home, assisted private home, assisted living facility, extended care unit, rehab department, 55+ senior care, or long-term care assisted living facility, as well as offer our in-patient unit.
Doctors – We have five M.D.’s on staff to provide hospice oversight.
Patients and families are usually anxious to return home. As you know, occasionally you send someone home at their wish, only to return to work the next day and see them readmitted.
We go the extra mile to make sure they have the care, equipment and support required; including continuous care, to allow them to remain at home should they choose.
Agape Home Care is State Licensed and Medicare Certified.
- State Licensed
All hospice agencies that provide “hands on” care in the home must be licensed by the GA ????. Through license, the state ensures the agency meets or exceeds specific standards of care. All hospice agencies must meet requirements for the administration the agency, staff qualifications, patient care, and patient records.
- Medicare Certification
Hospice agencies that wish to provide services and receive reimbursement under the Medicare or Medicaid hospice programs must be certified and meet federally mandated requirements, referred to as Conditions of Participation (COPs). Certified hospice agencies are subject to annual surveys to ensure the agency continues to be in compliance with all of these requirements.
Accreditation is a voluntary process in which a non-profit professional organization conducts a survey of the hospice agency. If the hospice agency meets the standards established by the organization, then the agency is granted “accreditation”. A hospice agency may seek voluntary accreditation from one or more of the three accreditation organizations: JCAHO, ACHA, and CHAP.
